The AW8 form is what you complete to claim your pension with the NHS.

It is a 14 Section form, the good news is you, as an employee, only need to complete Sections 6 – 14, your employer will do the rest.

The last four pages (13 – 16) are Guidance Notes only, there is nothing else to do except send the Form – ALWAYS remembering to take a copy for yourself, to your HR Department (or third party provider who provide your Payroll/Pension Services if guided to do so).
